Survey on PPE Needs and Concerns at Your Facility

To best grasp the evolving PPE situation in each of our facilities, we are asking our members to complete a very brief survey. The survey will ask:

  • your contact information
  • your facility (unless your hospital has its own survey below)
  • which PPE materials your facility is lacking
  • an open-ended question about concerns regarding PPE policy

The need for sufficient PPE and hospital management that supports increased masking standards is increasingly urgent. We thank you in advance for providing information that will aid us in making YOUR workplace safer.
